Admission Requirements

New students:

1. Complete Application
2. Birth Certificate- original
3. Social Security Card- original
4. Proof of Residency. Example: utility bill 
5. Immunization Card- signed by a doctor
6. Transcript- complete transcript including test results  
7. Additional records will be requested
8. Parent or guardian must supply photo identification
   

Returning students:

1. Returning application
2. Immunization Card- updated
3. Proof of Residency- Example: apartment lease
4. Parent or guardian must supply photo identification

Age Requirement

Kindergarten students must be 5 years old by September 1.

Uniforms:

MeyerPark is a uniformed school.  Uniforms are supplied by Parker School Uniforms, 713-661-6110.  Call the school office to ask about uniform donations or discounts.
